El Kinesiotape funciona?


El Kinesiotape funciona?

Introducción





De un tiempo a esta parte hemos asistido a la aparición de vendajes realizados con tiras de "esparadrapo" de llamativos colores colocados en los músculos y articulaciones de numerosos deportistas, y posteriormente en todo tipo de población.

Los pioneros de este tipo de vendajes se remontan a los años 70 en Asia con los Drs. Kenzo Kase y Dr. Murai, y no es hasta final de los años 90 cuando llegan a Europa a través de Alemania y Holanda con el futbolista Alfred Nijhuis, y posteriormente a España hacia el año 2000. Este tipo de vendaje es utilizado por los fisioterapeutas y se llama Kinesiotape o más concretamente Vendaje Neuromuscular (VNM)



Mecanismo de acción


El VNM está perfectamente indicado en patología músculo esquelética.

Y esto es así porque:

- Ayuda a la función muscular (tonifica o relaja, dependiendo de la técnica utilizada)
.
- Ayuda a a la función articular y corrección postural.

 Ayuda a la regeneración tisular.

Además tiene efectos neurorreflejo, analgésico y de activación de la circulación linfática y sanguínea.

Aplicaciones del VNM


El campo de aplicación del VNM es la patología músculo esquelética:

- Está especialmente indicado en personas que realizan Actividad Física y por supuesto en Deportistas.

- Es una técnica que puede realizar el fisioterapeuta de forma aislada, o bien complementando otras técnicas en el campo de la Fisioterapia y Ospetopatía.

Algunos tipos de VNM:




Pata de ganso: Utilizamos este vendaje como complemento de un tratamiento previo.
Sirve para relajar las inserciones musculares en la cara medial (interna) de la tibia, por debajo de la rodilla, conocido como "pata de ganso".
Es una patología muy frecuente en deportes de carrera: atletismo, fútbol, voley...



Tibial posterior: El tibial posterior es el músculo encargado de estabilizar la bóveda plantar, así como realizar adducción del tarso y flexión plantar. Se lesiona frecuentemente en corredores de larga distancia por su tracción a nivel de la insercion proximal en la tibia, provocando inflamación del periostio, a nivel de la inserción distal en el escafoides, cuñas y 2º, 3º y 4º metatarsianos.



Lumbalgia: VNM en forma de X o "estrella" para tratar puntos de dolor localizado (L4 y L5 en este caso), como método de descompresión local.

Funciona el VNM?


Efectivamente existe mucha evidencia científica que aporta los beneficios que ofrece el VNM, especialmente en el campo de la patología músculo esquelética, además de la práctica clínica diaria de los fisioterapeutas que tienen formación en la aplicación correcta de esta técnica.

Aplicaciones en Medicina deportiva

Introducción

El Plasma Rico en Plaquetas (PRP) está teniendo una extraordinaria difusión en el campo de la Medicina Deportiva, así como en otras especialidades médicas.

Se usa en la cimentación de los implantes, y como preparación del lecho mandibular donde posteriormente se harán los implantes, pues tiene efecto osteogénico y "engrosa" la mandíbula del paciente con osteopenia (poco hueso) normalmente de edad avanzada y el tornillo del implante no "agarra" pues no se puede introducir en profundidad por tocar el nervio maxilar.

En otras especialidades como es la Medicina Estética el PRP se usa como elemento activador del fibroblasto produciendo el efecto de "bioestimulación facial" cuando se aplica en mesoterapia en cara, cuello y escote, favoreciendo la densidad de la matriz dermica, aportando mayor luminosidad a la piel, y consiguiendo un efecto lifting de las zonas tratadas. (será tema de otra entrada en este blog).

Medicina Deportiva

El Plasma Rico en Plaquetas (PRP, también llamado Platelet-rich plasma in growth factors (PRGF), es usado en Medicina Deportiva para tratar lesiones musculo-esqueléticas agudas y crónicas, así como en procedimientos quirúrgicos en hombro, cadera y rodilla, incluyendo cimentación en prótesis de cadera, reconstrucción de LCA en rodilla, y mejorando los mecanismos de osteogénesis.
Actúa sobre el dolor, disminuyendo la dosis de analgesia que precisa el paciente en cirugía de descompresión subacromial.

En Medicina Deportiva lo usamos con éxito en las siguientes patologías:

-  Tratamiento conservador de una rotura muscular (en una entrada reciente hemos explicado el tratamiento de la lesión de un gemelo en la pierna de un jugador de rugby) aplicando el PRP en forma líquida mediante infiltración ecoguiada en el punto lesional,

-  Tendinitis y tendinosis aplicando PRP en forma líquida en el cuerpo del tendón o en la zona adyacente (ejemplo grasa de Hoffa en rodilla), siempre guiados con la Ecografía

 - Lesiones de cartílago en rodilla, normalmente son casos de condromalacia en deportistas de todas las edades y disciplinas. Otras veces son casos de artrosis de rodilla en adultos y 3ª edad que mejoran con la aplicación del PRP infiltrado intrarticularmente en forma líquida.

- Lesiones de cadera, principalmente son casos de artrosis de cadera, donde vamos a infiltrar el PRP en forma líquida, aquí es imprescindible hacerlo con el Ecógrafo para acceder al interior de la articulación.

Tratamiento con PRP en deportistas: Posición del COI (Comité Olímpico Internacional)

El COI elaboró un Documento de Consenso en el año 2010, (Br. J. Sports Med 2010 44 1072-1081, doi 10.1136/bjsm.2010,079822) mostrando su posición, que resumimos a continuación:
En 2010, el PRP fué nombrado de forma específica apareciendo en la lista de sustancias prohibidas, siendo prohibido en inyección intramuscular y permitiendo las aplicaciones de PRP vía intra-articular, intra y peritendinosa con una Declaración de Uso. 

Se pensaba que el PRP podria contener factores de crecimiento que estimularían células musculares satélites y aumentar el tamaño y la fuerza de los músculos tratados. Estas consideraciones no han sido documentadas clinicamente, como tampoco se han documentado reacciones adversas (fibrosis, infección, carcinogénesis) cuando el protocolo es efectuado correctamente, con procedimiento y material ajustado a la normativa sanitaria vigente.

Posteriormente la prohibición del COI y de la WADA para el uso de PRP infiltrado intramuscularmente fué eliminada de la Lista de Sustancias y Métodos Prohibidos en 2011.

Actualmente el PRP inyectable está permitido por el COI y por la WADA para ser administrado de forma terapéutica en deportistas por cualquier vía de adminitración.

Obra imprescindible

El libro que os muestro es una obra imprescindible que debe estar en la mesa del médico especialista en ecografía musculoesquelética. También es de lectura obligada para el médico y fisioterapeuta que quiera acercarse al diagnóstico por imagen a través de U.S.

Escrito por dos prestigiosos médicos radiólogos: Dr. Ángel Bueno Horcajadas del Hospital Universitario Fundación Alcorcón, Madrid; y el Dr. José Luis del Cura Rodríguez, Profesor Asociado de la Facultad de Medicina de la Universidad del País Vasco, y Jefe de Sección del Servicio de Radiodiagnóstico del Hospital de Basurto.

Además el Dr. del Cura es profesor habitual de los Cursos que se organizan anualmente en el Centro de Alto Rendimiento de San Cugat del Vallés, donde he tenido el privilegio de asistir a 3 ediciones, y comprobar como se puede hacer "fácil" una exploración compleja cuando es guiada por un profesional de este prestigio. (aun recuerdo como nos enseñó a quitar una calcificación en el supraespinoso a base de infiltrar una solución salina pinchando la calcificación, y a su vez proyectando la intervención en directo por video en el salón de actos del CAR. sublime!).

La obra se divide en 11 capítulos, el 1 para introducción a la ecografía y del 2 al 8 se tratan por regiones: hombro, codo, muñeca y mano, muslo y cadera, rodilla, tobilla y pié, las referencias anatómicas, la sistemática de exploración y la patología de las zonas estudiadas. Todo con esquemas anatómicas, fotografías e imágenes ecográficas.
El capítulo 9 se dedica a la ecografía del nervio periférico, el 10 a los tumores de partes blandas, y el capítulo 11 al intervencionismo guiado por ecografía, escrito por Dr. del Cura y Dr. Zabala.

La obra cuenta con un acceso web, en el que previo registro, permite complementar las imágenes del libro con animaciones, figuras y videos asociados a cada capítulo.
Además cuenta con el aval de la Sociedad Española de Ultrasonidos (SEUS).

Tendinitis rotuliana en Piragüista

Tendinitis rotuliana. Imágenes Ecográficas.

Historia Clínica: (10/01/13)

Piragüista de alto nivel que presenta dolor en rodilla izda. después de realizar actividad física: running y también los días que hace "agua", (en la piragua lleva ambas rodillas en semiflexión y haciendo un efecto de "pedaleo" acompañando al gesto del paleo, la pierna y el brazo contralateral), dolor desde hace 1 mes.

Exploración:

Al presionar el tendón rotuliano presenta dolor y crepitación.


Ecografía del tendón rotuliano:

Zonas hipoecogénicas en el tendón rotuliano, debajo del polo inferior de la rótula. Power doppler negativo.
El tendón presenta un grosor normal:


Diagnóstico:

Tendinosis rotuliana

Tratamiento:

Reposo relativo, tratamiento de fisioterapia convencional (US, láser  masaje transverso, kinesiotape), Ibuprofeno 600mg.

Revisión y Ecografía (08/02/13)

Continúa dolor a la presión en polo inferior rótula
Presentamos la comparativa con el tendón rotuliano contralateral. (el tendón problema lo situamos a la izda.) La inserción en el polo inferior de la rótula aparece hipoecogenica y pierde la continuidad en la cortical. El grosor es normal.


Se amplía el tratamiento rehabilitador anterior con iontoforesis (fortecortin) y ejercicios excéntricos.

Revisión y Ecografía (28/02/13)

Mejoría clínica. Puede realizar carrera contínua sin dolor. No hay dolor a la presión.
La imagen ecográfica es normal, muy similar a la contralatera. El tendón quizás es de grosor ligeramente superior a lo normal (suele ser inferior a 0.4cm) pero también ocurre en el contralateral por lo que lo informamos como normal.
